Office Policies
The following is a copy of the office policies you will be asked to sign when you arrive for your appointment.
1.) Insurance information must be presented and verified prior to your appointment. In the event my insurance provider determines that I am not eligible for visual insurance coverage or eligible for a reduced level of coverage, by signing this statement I hereby agree to be financially responsible for any and all charges I incur that are not paid by my insurance provider.
2.) I acknowledge receipt of a copy of Focal Point Eyecare’s Notice of Privacy Practice.
3.) I understand all fees for services and products rendered are due and payable at the time of service.
4.) I understand fees paid for services are non-refundable and that all sales are final. No refunds are available for lenses or any package deals. A frame return will require a restocking fee of $65 before any refunds are issued. Contact lenses may be returned for a refund if packages are unopened and unmarked. All returns must be within 60 days of purchase.
5.) I understand that I have access to a digital copy of my spectacle prescription through my online portal. If I am being evaluated for contact lenses, my contact lens prescription will be availalbe in my portal after being finalzed. Contact lens prescriptions must be finalized within two follow-up visits and 60 days of the initial fit. Additional charges apply with additional visits or after 60 days.
6.) Glasses prescriptions must be finalized within 60 days of initial exam. Additional charges apply after 60 days.
Focal Point Eyecare Institute of Myopia Management may ask to use your images for training or to post images on social media and the office website. You will be allowed to select how much exposure you would like, if any, when signing your annual policy.
